Our mahi is to provide support for whanau living with long-term conditions, this includes support for managing the effects of the condition, engaging with activities which enhance wellbeing and navigating some of the barriers to service that whānau encounter.

You may be referred to our services by your GP. We are also happy to take self-referrals from whānau, follow the link below and complete the self referral form.
